If you have received a letter from the Ministry of Transportation regarding your ability to safely operate a motor vehicle based on your medical condition, our company can conduct an independent medical assessment. Our independent medical assessments are approved by the Ministry of Transportation.
Our staff is fully licensed and certified to conduct these assessments. Our doctor is a physician who specializes in addiction medicine and our addiction counsellor is an internationally certified alcohol and drug counsellor.
The assessment process is a self pay third party uninsured medical service. It is very important to understand that the Independent medical assessment is not a purchase of your license back from our addiction physician.
Setting up an Appointment

When you contact our office, please have your letter or letters that were sent to you by the Ministry of Transportation. We have included a brief description of how the process works.
After you contact the office and we have a copy of the document from the Ministry of Transportation, we will send you a package contains several documents; an introduction letter, a consent form, an agreement for compensation form, a laboratory slip if required by the Ministry of Transportation and a self addressed stamped envelop.
Once you receive the package, you need to sign the forms and go to the lab to have the tests performed. The forms can be sent back in the envelope provided. The payment for the services can be sent in the same envelope. We accept major credit cards, money orders and certified cheques. Please do not send cash in the mail. If you are paying by credit card, we can process that payment over the phone.
The fee is fourteen hundred dollars, $1400.00 for the complete assessment, lab work, and report sent to the ministry of transportation on your behalf.
